Case review discussion for advice by an Occupational Health Doctor with the employer (Manager/Human
Resources). Can be carried out at our clinic or via video/telephone call.
Heavy Goods Vehicle (HGV) or PCV - £215
Includes Medical History, Examination and completion of DVLA form D4.
HSE Ionising Radiation - £250
Face to face consultation including medical history, examination and completion of certificate.
Hand Arm Vibration Tier 4 Assessment by Doctor - £345
Medical face to face assessment, with report and diagnosis.
